How do I register my child?

All registration forms for the 2019 season can be found after you click the registration button below. Please make sure to bring in all completed forms at the time of registration.

The Pop Warner Little Scholars organization has a clear focus on the safety and academics of its participants. As a result, there are a few “mandatory” forms that must be completed in order to register your child. Below is a summary of all the necessary paperwork.

The following items are needed for registration:
  • Mandatory online registration and non-refundable deposit
  • Parent/legal guardian and participant must be present at registration
  • Completed registration forms:
    • 2019 Sun Devils Parent Code of Conduct
    • 2019 Pop Warner Participant Contract and Parental Consent Form
    • 2019 Pop Warner Physical Fitness & Medical History Form
      • Section I is completed by the parent/guardian
      • Section II is completed by a physician ONLY
    • 2019 Photo & Video Release – NEW!
  • Official Birth Certificate – Bring original and make a copy of your birth certificate and bring both with you to registration. Returning 2018 Sun Devil players/cheerleaders DO NOT need to bring a birth certificate

* 2018-2019 end of the year report card will be required before any child will be eligible to participate. This will be collected prior to participation.

All physicals must be performed and signed in the year 2019

There will be free physicals onsite for each player on the following dates:

  • Tuesday, June 18

No physicals dated or performed in 2018 will be accepted. This includes the date the physical was performed on the child. All physicals must be done on the Pop Warner physical form NOT a Sarasota County School form.

Also, NO FAX COPIES will be accepted. League rules require the form to be the original document. Any document containing “white out” will not be accepted and a new form will have to be completed.

Please register via a credit card through Affinity. (Registration requires full payment of $345). $100 will be non refundable.

*If you currently have a profile in Affinity and you are registering for the new season, please delete and recreate it to ensure accuracy.

Once you log in, you will need to go to My Account and select “View Payment” for the child you are registering. Click “Edit Payment for Fall 2019” and make the payment.
